日本人科学者が数十年の鎌状赤血球症研究で成果

日本生まれの医師で研究者の新原豊博士が指揮するエマウスライフサイエンス社(Emmaus Life Science、以下同社もしくはエマウス社)は成人、小児の鎌状赤血球症の治療薬の販売承認を求める予定である。承認されれば、この治療薬は約20年間で初めての新しい鎌状赤血球症治療薬になる。鎌状赤血球症は最も破滅的な遺伝性疾患で痛みの症状と若年での死を伴う。この治療法は医薬品グレードのL-グルタミンの経口投与である。

エマウス社は9月に鎌状赤血球症治療薬についての新薬申請(NDA)をFDAに提出する予定であり、これが鎌状赤血球症の小児患者向けにFDAが初めて承認する治療薬になる可能性がある。

エマウス社の会長兼最高経営責任者(CEO)である新原豊博士は「NDAの提出は多くの人々のライフワークの成果であり、この達成のために寄与したすべての患者、研究者、投資家、パートナー、支持者、従業員にとってのマイルストーンである。NDA提出は来月初めを目指しており、試験の前向きの安全性、効能のデータについてFDAと協議する」と語っている。

鎌状赤血球症に対しては広く利用可能な治療法、受容性の高い治療法がないため、治療法の開発がインドをはじめ世界全体で優先事項だった。このためインドのモディ首相は2014年の訪問の際に日本の政府、科学界に対してこの病気からの救済策の発見を呼び掛けたほどである。

世界で毎年30万人以上の子どもが鎌状赤血球症を持って生まれており、世界で約2500万人がこの病気で苦しんでいる。診断や治療を受けなければ、サハラ砂漠以南のアフリカで鎌状赤血球症(SCD)の子どもの90%は生後5年以内に死亡する。

新原博士はさらに「医師として私は鎌状赤血球症の人の非常な痛み、苦しみを見てきた。これが治療法の発見へとわれわれを駆り立てた。われわれが鎌状赤血球症について行った最大の試験である米国での臨床試験で得たデータについて、われわれは確信を持っている。特に私は鎌状赤血球症共同体とわが社にとってのこの重要なマイルストーンの達成を助けてくれた多くの人々への感謝を表明したい」と述べている。

同社の第III相鎌状赤血球症試験のデータは、鎌状赤血球症の症状発現と入院の回数の減少、累積入院日数の減少、生命を脅かす急性胸部症状の発生回数減少を示した。この臨床試験は成人患者と5歳以上の小児患者230人を対象に、米国の31カ所の経験豊かな鎌状赤血球症治療センターで行われた。高尾の治療法が原因の大きな悪性事象はなかった。

エマウス社はこれまでに日本、韓国、香港、米国の投資家の支援に加え、FDA、米国立衛生研究所からの資金援助で約5000万ドルの資金を集めた。鎌状赤血球症の研究とともにエマウス社は米国で輪部幹細胞欠損向けの上皮細胞シート角膜再生技術を商業化するため東京に本社のあるセルシード社と提携している。

▽鎌状赤血球症について

鎌状赤血球症は遺伝性の血液障害で、赤血球が酸化されて硬直した鎌状になり小血管をふさぐ。この病気は衰弱性の痛みの発作と臓器の損傷を起こし、若年での死亡を招く恐れがある。鎌状赤血球症は米国では約10万人、欧州連合(EU)では推定8万人、世界では2500万人がかかっている。この病気は特に祖先がサハラ砂漠以南のアフリカ、南米、カリブ海地方、中米、中東、インド、地中海地方を含む地域から来た人々の間で起こる。

Japanese Scientist Culminates Decades of Research on Sickle Cell Disease

   Emmaus Life Sciences, led by Japanese born physician researcher Dr. Yutaka

Niihara, plans to seek marketing approval for a treatment for adult and

pediatric sickle cell disease. If approved this treatment would be the first

new sickle cell disease treatment in nearly 20 years. Sickle cell disease is

one of the most devastating hereditary diseases with painful symptoms and

shortened life span. The treatment consists of orally administered

pharmaceutical grade L-glutamine.

   In September, Emmaus is expected to submit a New Drug Application (NDA) to

the U.S. Food and Drug Administration (FDA) for the company’s treatment of

sickle cell disease in what also potentially could become the first FDA

approved treatment ever for pediatric patients with sickle cell disease.

   “The submission of the NDA is the culmination of many people’s life work,

marking a milestone for all the patients, researchers, investors, partners,

advocates and employees who have contributed to this achievement,” said Yutaka

   With the lack of a widely available or well-tolerated treatment for sickle

cell disease, developing a treatment has been a priority throughout the world,

including in India.  It led to an appeal by Prime Minister Modi of India to the

Japanese government and scientific community to find a remedy for the disease

during his 2014 visit.

   More than 300,000 children worldwide are born every year with sickle cell

disease, and approximately 25 million suffer from the disease worldwide. If

undiagnosed or untreated, 90 percent of children with SCD in sub-Saharan Africa

will perish within the first five years of life.

   “As a physician, I witnessed the extreme pain and suffering of those

afflicted with sickle cell disease, which drove us to finding a treatment,”

continued Dr. Niihara. “We are confident in the data from our clinical trial in

the U.S., the largest such trial to date to have been completed for sickle cell

   Data from the Company’s Phase 3 sickle cell disease trial demonstrated a

reduction in the frequency of sickle cell crises and hospitalizations, as well

as a reduction in cumulative days hospitalized and a lower incidence of the

life-threatening acute chest syndrome. The clinical trial enrolled 230 adult

and pediatric patients as young as five years of age, across 31 experienced

sickle cell disease treatment centers in the United States. There were no major

adverse events attributable to the treatment.

   Emmaus has raised over $50 million to date, with backing from investors in

Japan, Korea, Hong Kong and the United States, as well as grants from the U.S.

FDA and U.S. National Institutes of Health. Along with its work in sickle cell

disease, Emmaus also has a relationship with Tokyo-based CellSeed, to

commercialize Epithelial Cell Sheet Corneal Regeneration technology for Limbal

Stem Cell Deficiency (LSCD) in the United States.
